To report fraud, use the official channels
We are a news site, so a report sent to us does not reach investigators. Send it to the channel that fits your case.
Your insurance company
Most insurers have a special investigations unit for fraud. Use the fraud line or page your insurer lists, and keep a copy of what you send.
Your state insurance department
Your state's insurance department, or its fraud unit, takes reports of insurance fraud in your state. The NAIC's online system sends a report to the right state agency.
National Insurance Crime Bureau
Takes tips on suspected insurance fraud by phone (weekdays, 7 a.m. to 7 p.m. Central) or online, and you can stay anonymous. It does not handle complaints about an insurer's service or rates.
HHS Office of Inspector General
The federal health watchdog's hotline takes reports of fraud in Medicare and other federal health care programs. Medicare also takes reports at 1-800-MEDICARE (1-800-633-4227).
Your state's Medicaid Fraud Control Unit
Each state has a unit that investigates and prosecutes Medicaid provider fraud and the abuse or neglect of residents in care facilities. HHS-OIG lists every unit.
DHS Office of Inspector General
Takes reports of fraud involving FEMA disaster aid and other Homeland Security programs. For a contractor or claim scam after a storm, use NICB.
More official channels
- Health care fraud, any insurance: The FBI investigates health care fraud against both federal and private insurance, and its health care fraud page asks you to file a complaint at ic3.gov. File at ic3.gov
- Workers' comp: Report workers' comp fraud to your insurer or your state's insurance fraud unit. The U.S. Department of Labor lists every state's workers' compensation agency. Find your state's agency
